Our Vision

Activated neighbourhoods participate in compassionate support of one another making every neighbour feel supported in his or her community.

Our Mission Statement

To promote physical, spiritual and psychosocial well-being of older adult neighbours living with advanced chronic conditions through neighbourhood activation and fueled by the partnership of a network of retired, non-practicing and practicing health professional volunteers.

Our Society’s Objectives
  • To facilitate activation of neighbourhoods in the compassionate care of one another.
  • To facilitate neighbourhood health related discussions.
  • To empower older adult neighbours living with advanced chronic conditions and their caregivers by offering to:
    • Be their health care advocate.
    • Provide health care system navigation.
    • Facilitate connections with health care and community resources.
    • Facilitate health literacy support.
  • To facilitate the preparation of health records and advanced care planning.
  • To promote public awareness of the Society and its work and, thereby, obtain financial support to carry out its purposes.
Our Society’s Statement of Values, Beliefs and Code of Conduct (abridged)
  • To promote, value, and cherish the sanctity of life and engage and support neighbours to live each day until the day of natural death.
  • To provide equal advocacy and care for each person regardless of ethnic background, socio-economic status, race, religion, or gender.
  • To strive to be an active compassionate listener who is respectful and dependable.
  • To provide support that is person and family centered.
  • To honour confidentiality.
  • All support will be provided free of cost while upholding ethical business practices.
